MEDICATION THERAPY MANAGEMENT PHARMACIST

Position Description

This contracted Medication Therapy Management (MTM) Pharmacist position is offered to individuals with strong clinical and interpersonal skills, a genuine desire to connect with people, a congenial and positive personality, a sound working knowledge of MTM and other pharmacy provided cognitive services, and strong communication skills with physicians and all other health care professionals.

By providing MTM services to patients through telephonic consultations, the successful candidates will collaborate with physicians and other health care professionals to implement optimal medication regimen for patients, identify potentially harmful medication related problems, and optimize medication use for improved patient outcomes. The successful candidates will use their clinical knowledge to enhance patients’ understanding of appropriate medication use, increase adherence to medication therapy, improve detection of adverse drug events, promote the appropriate use of cost-effective medications and be comfortable with providing clinical recommendations to physicians and other prescribers involved in patients care.


Essential Duties and Responsibilities

  • Contact patients telephonically to conduct an MTM session
  • Develop and utilize successful interpersonal techniques that are effective in engaging a high percentage of providers and patients willing to engage in offered services
  • Analyze patient data and assess opportunities to improve and/or enhance care including but not limited to avoidance of adverse events, generic substitution, therapeutic interchange, and adherence to evidence based clinical practice guidelines
  • Effectively document clinical  services provided to patients on create quality cumulative data from individual services provided
  • Reach out via telephone and/or fax to health care providers of patients to communicate clinical recommendations and execute clinical services for these patients via appropriate interaction and intervention with their providers
  • Provide a variety of telephonic pharmacist-delivered patient care services  patients, including but not limited to medication therapy management

Skills and Requirements

  • Licensed pharmacist in Virginia, PharmD degree strongly preferred
  • Sound working knowledge of Medicare Part D MTM services
  • Congenial personality, warm telephone manner
  • Excellent written and oral communication skills
  • Ability to work under pressure and achieve deadlines
  • Ability to work with others in a collaborative environment
  • Ability to incorporate new technological  approach to pharmacy practice
  • Idea-oriented individual with an ability to problem solve and work independently
  • A dedicated desire to advance patient care and the profession of pharmacy

      Location: This position will require working in our Reston VA location. This is not a remote consulting pharmacist position.
